What Block 8 Captures
Block 8 identifies who will carry the waste and how. For sea freight this typically means the shipping line or freight forwarder. If a forwarding agent organizes the transport but does not carry the waste themselves, the agent's details go in Block 8 and the actual carriers are listed in an attached annex.

How to Complete Block 8

Enter the carrier's registration or licence number,
full legal name, full address (including country), contact person, telephone, fax, and email.

Enter the means of transport code.
Use multiple codes if the shipment uses more than one mode (e.g., R + S for road to port then sea freight).

If more than one carrier is involved,
append a complete carrier list as a numbered annex. Write "See attached list" in Block 8.

If a forwarding agent organizes the transport,
enter the agent's details here and list actual carriers in an annex.

Provide the best available information — typically the freight forwarder or shipping agent. The movement document (completed at shipment time) captures the actual carrier with signatures.
Does the carrier need to be registered with any authority?

Many countries require carriers of hazardous waste to hold a specific transport licence. Include the licence number in the registration field where applicable.
Who completes Block 8?
