- Must identify waste generator, exporter, carrier, importer, and disposer
- Specifies disposal or recovery operations to be performed
- Includes liability provisions and insurance requirements
- Addresses illegal trafficking scenarios and re-import responsibilities

FACILITY OPERATING PERMIT OR LICENSE:
- Current permit from receiving facility
- Permit must explicitly list Basel waste codes in notification (Y49, A1181, etc.)
- Must authorize specific operations (recovery or disposal codes)

- Certified copy or original depending on competent authority requirements
