What Block 10 Captures

Block 10 establishes the legal identity of the receiving facility — the specific authorized site where the waste will be disposed of or recovered. Competent authorities use this block to verify that the declared destination holds a valid permit for the declared waste type and operation. The facility declared here must remain valid for the entire consent period — not just at the time of notification.

How to Complete Block 10

Tick either Disposal Facility or Recovery Facility.

These are not interchangeable and must match Block 3 (B(i) or B(ii)) and Block 11.

Enter the facility's registration or permit number

as issued by the competent authority in the country of import.

Enter the exact legal name

as it appears on the facility's authorization. Do not use trading names or abbreviations.

Enter the full address.

If the actual processing site differs from the registered address, enter the registered address here and complete the

Actual site of disposal/recovery

sub-field.

If the disposer or recoverer is also the importer (Block 2),

write

"Same as Block 2"

.

Interim Operations (R12/R13, D13–D15)

If the declared operation in Block 11 is an interim code, attach an annex identifying all subsequent facilities and the final operation code where actual processing is completed.

Frequently Asked Questions

Can the facility change after the notification is approved?

Only through a formal variation process managed by the competent authority. Routing waste to a different facility without prior approval is a critical compliance breach.

What if the facility is also the importer?

Write "Same as Block 2" in Block 10. No need to repeat all contact details.
